If you are choosing to pay by invoice, please ensure you have the correct details of the person / department who will be responsible for making payment.  

Please note this booking form is intended for single bookings only. If you wish to book additional places, please submit a separate form for each attendee.

As part of this form, we will ask for details that can be used to identify you –  e.g. your name, email address, job title, institution, location, etc. 

We will store all your responses securely in password-protected files held on a server hosted by the University of the West of England, Bristol. Your responses will be stored for 12 months from the date of the training, after which time they will be permanently destroyed.
