We would like you to let us know how you wish to proceed with your involvement in the Domain of Action Groups. 

You may wish to proceed as you have been doing, switch to another group or remove yourself from the process at this stage. 

Honoraria

We will be offering a set honoraria to those who's employers are unable to support your participation in this project. This will be for those who are self-employed, freelancers, unemployed, or who are doing this over and above their usual working hours. 

We have used guidance from the NIHR and the NHS to inform the amounts awarded and will offer monetary recognition of the time you volunteer towards this project. 

As part of this form, we will ask for details that can be used to identify you –  e.g. your name and email address. This information is needed so that we can know which Domain you are part of and would like to continue working with.

We will store all your responses securely in files held on Microsoft services hosted by the University of the West of England, Bristol. The data are accessible only to members of the NCCPE; access requires multifactor authorisation using institutional names and passwords.   Your responses will be stored for 3 years from the survey completion date after which time they will be permanently destroyed.
